Description & Services
DescriptionThe Healthy Babies Healthy Children program consists of 4 components. 
1) Screening in the hospital to identify any possible concerns which might affect the health of the mother or the growth and development of the newborn. The hospital sends this screen to the Health Unit with consent. 
2) Telephone contact by a Public Health Nurse to mothers of new babies within 48 hours of discharge or first work day whichever comes first to offer a listing of resources and services available to the new family. 
3) Offer of a home visit by a Public Health Nurse to all families of newborns. 
4) Public Health Nurses may offer the services of a Family Home Visitor.  
Family Home Visitors are experienced parents who are trained to assist families meet the needs of their children. 
Supports a family home visitor provide include: 
* Pre-natal information, 
* Information about child development and safety,  
* Parenting support,  
* Information about community programs such as food resources, support groups, housing assistance and subsidized child care,  
* Translation and interpretation of information for families.  
Healthy Babies, Healthy Children is focused on parents with children birth to 6 years of age and prenatal

ApplicationOriginal referral comes from hospital staff. Self referrals may be made if not contacted within 48 hours by a Public Health Nurse after hospital discharge * Physicians and social agencies involved with the family may also refer
