Mon-Fri 8:30 am-4:30 pm

  • seniors mental health worker available throughout the week however Geriatric Psychiatrist available for direct consultation approximately two days per month
Areas Served Brant County ; Brantford
Eligibility Ages: 65 year(s) and up Referral criteria
  • older adults with behavioural disturbances and/or mental health problems associated with cognitive impairment
  • mental health problems accompanied by significant medical illness and/or functional needs
  • chronic mental illness who develop complicating disorders arising from an age-related decline in physical and mental health
  • late-onset severe mental illness
Application

Open referral, however, requires physician approval. Primary physician must be involved

  • long-term care referrals are to be faxed to 905-381-5613
  • community/retirement home referrals to be faxed to Connect (centralized intake service for St. Joe’s Mental Health and Addiction outpatient programs) at 905-389-3815, using the referral form found here

Description (Service)

Regional service to assist family, community and long-term care providers in the continuing care of older adults with complex cognitive, emotional and behavioural disorders

  • specialized assessment, consultation, treatment and short-term specialty case management and intervention
  • education and community/program development, community homes, retirement, and supportive housing and long-term care facilities

Wait times can be upwards of 5-6 months to see a psychiatrist
