Description & Services
Description (Service) First point of contact for children and youth ages 0 - 18 with mental health concerns, intellectual disabilities, or Autism * first place to contact for information, questions, and access to community resources and programs.  
 
Help navigate services available to children, youth, and their families * complete an intake for children or youth experiencing behavioural or emotional concerns to get the services they need * sends referrals to community services that help children, youth and their families deal with concerns such as: 
* anxiety 
* depression 
* drug and alcohol abuse 
* grief 
* divorce 
* self-esteem 
* bullying 
* sexuality 
* self-harm 
* fire-setting behaviours 
* anger 
* aggression 
* acting out behaviours 
* family dynamics 
* learning disability 
* excessive worrying 
* disruptive behaviours  
 
Also complete an intake and referral for children and youth with an intellectual disability and their caregivers to connect to supports that can help them participate in the community, further skill training, provide respite, or develop strategies for successfully living with behavioural or mental health concerns 
 
Lead Service Coordination agency in Brant - offering coordinated service planning to families to ensure there is a clearly identified staff for each family to lead their service plan and support services working together to meet each families' unique needs 
 
Assists parents with parenting concerns including referral to parenting programs including but not limited to the Triple P (Positive Parenting Program) as well as to information on community resources available to adults and families 
 
Manages the Community Information Brant, Haldimand & Norfolk database info-bhn.ca containing information on local community programs and services
